Please feel free to call us ahead of time to check if we are running late.

Please inform the reception staff as soon as possible if you need to cancel or reschedule your appointment. A fee may apply for late cancellations.
Referrals
In order to claim for a Medicare rebate for your consultation, you will need to bring a valid referral from a general practitioner, optometrist or another specialist. Referrals from general practitioners and optometrists are usually valid for 12 months. Referrals from specialists are valid for 3 months.
New Patients
Prior to your appointment we will send you a link to our Patient Registration Form. Once completed, it will automatically upload to our patient management system.
Alternatively, you may click the button below to download the Patient Registration Form and complete it prior to your visit:
On the day of your appointment, please bring:
- Referral
- Medicare card
- Private health insurance details
- Government pension card
- Department of Veteran Affairs – pension card or Gold Card
- Current glasses
- List of medications
- Any relevant tests
Please allow at least 1.5 -2 hours for your initial appointment. At Glen Iris Eye Specialists, an orthoptist and an ophthalmologist work collaboratively to assess your eyes. Initial testing will be with an orthoptist.
Most patients require pupil-dilating drops to check the inside of the eye and assess a child’s optical status (need for glasses). The drops can take up to 30 minutes to work or longer if you have dark brown eyes, after which you see your ophthalmologist for further assessment and discussion regarding treatment.
It is advisable that you do not drive after your pupils have been dilated until your vision returns to normal (usually 1 -2 hours).
We recommend that you bring sunglasses to your appointment as you will be light sensitive.
Fees
Glen Iris Eye Specialists is a private billing practice. Payment is required on the day of consultation. Medicare rebates are available and we can process your Medicare claim online. You will be advised of your initial consultation fee before your appointment.
Concession fees are offered to Government Pension card holders.
Patients with a Veterans Affairs’ Gold Card are fully covered. For White Card holders, you will need to check your specific entitlements to confirm if ophthalmology is covered.
Payment is due on the same day as your appointment. We accept cash, eftpos and credit cards – including AMEX and Diners.
